For Patients

For your visit, you will need

1. A valid Medicare card
2. A referral from a GP or specialist to claim rebates from Medicare
3. Any test results you have had which relate to this problem, CT/MRI scans (actual scan images),  audiograms, pathology test results, sleep studies etc. Sometimes it will save repeating these tests.
4. Completed patient registration form including a list of your current medications and allergies (sent with your appointment confirmation)
5. Current Veteran’s Affairs, pension or health care cards where applicable
